Holism Inc. ("Holism," "Holism Health," "we," "us," or "our") operates a holistic health marketplace and practice management platform (the "Platform") that connects Patients with holistic, integrative, and complementary medicine Practitioners.
This Notice of Privacy Practices ("Notice") describes how we may use and disclose your Protected Health Information ("PHI") and outlines your rights under HIPAA.
This Notice applies to PHI that we create, receive, maintain, or transmit in connection with the clinical and care-related features of our Platform.
2. Our Role Under HIPAA
Holism generally operates as a Business Associate to Practitioners on our Platform, who are Covered Entities under HIPAA.
- We handle PHI on behalf of Practitioners under Business Associate Agreements (BAAs)
- In certain cases, where Holism independently determines how PHI is used, we may act as a Covered Entity
3. How We May Use and Disclose Your PHI
3.1 Treatment
We may use and share your PHI to support your care.
3.2 Payment
We may use your PHI to facilitate payment for services.
3.3 Healthcare Operations
We may use your PHI to operate and improve the Platform.
3.4 Business Associates
We may share PHI with trusted service providers who perform functions on our behalf.
3.5 As Required by Law
We may disclose PHI when required by federal, state, or local law.
3.6 Public Health and Safety
We may disclose PHI to prevent or lessen a serious threat to health or safety.
